Healthcare outsourcing and BPO

Human plus AI delivery your clients can audit

We help back-office and administrative support providers redesign delivery so AI handles the repeatable steps, people handle exceptions, and clients can see quality in the numbers.

Short answer

How can AI help a healthcare outsourcing or BPO provider?

AI can take on repeatable steps inside client workflows, such as sorting incoming documents, drafting appeals, checking eligibility and preparing status reports, while trained staff handle exceptions and quality sampling. The value comes from a redesigned delivery model with logged outputs, not from adding a tool to an old process. Client data safeguards and contracts come first.

Who this covers

Organizations in this segment

  • Revenue cycle and coding service providers working as a vendor to provider groups
  • Outsourced patient access teams, answering services and scheduling call centers
  • Prior authorization, referral and records processing back offices
  • Transcription, documentation and virtual assistant firms serving practices
Challenges

The business problems that hold this segment back

01

You are paid for people, clients buy outcomes

If AI shortens the work behind a task, per-hour or per-FTE pricing starts to look wrong to the buyer. In Bessemer and Bain's September 2026 sample, half of surveyed organizations reported cutting headcount because of AI or planning to within six months.

02

AI lengthens the chain of custody for client data

HHS says an AI-enabled contractor handling PHI for a covered entity can be a business associate, and names an AI patient-portal chatbot as a possible example. Every AI vendor you add is another link a client will ask about.

03

Buyers outsource revenue cycle work, and many have not yet adopted AI in it

In a 2026 Guidehouse and HFMA survey of provider executives, 69% outsourced some or all revenue-cycle work, yet 59% had not implemented AI or automation in their revenue cycle. These are buyer figures, and they matter to billing and RCM companies too.

04

Quality is invisible unless you measure it

Clients see the final claim or call outcome, not your checks. An AMA survey reported in May 2026 found only 24% of physicians said medical-necessity denials were consistently reviewed by qualified clinicians, so expect clients to ask for proof.

05

Every client runs different systems and rules

The same eligibility check runs through different EHRs, payer portals and fax queues. McKinsey found integration the most-cited barrier to scaling AI, which makes data integration a delivery cost, not an afterthought.

Where AI fits

Suitable AI, and how mature each application is

Established means widely deployed and well documented. Emerging means real deployments exist but results vary with your data and workflow. Early means pilots or research, not a safe default.

Established

Fax and referral packet intake

Scanned pages are classified, fields extracted and patients matched, with unmatched items sent to a review queue. Patient matching is never perfect on messy faxes, so people still work the remainder and own the exceptions. AI automation keeps the source image linked to every extracted field.

Established

Coding support and coverage checks

Defined back-office tasks such as documentation checks, code suggestions and coverage checks have task-level evidence. A coverage check is not an authorization decision, so a coder or specialist still approves anything that changes a claim.

Emerging

Denial and appeal drafting

Appeal letters are drafted from the payer's denial reason and the supporting documents, then edited by an appeals specialist. Use is still limited, though affected payers generally must give specific denial reasons from 1 January 2026 under CMS-0057-F.

Emerging

Risk-weighted quality sampling

AI picks which completed items an auditor should check and groups errors by cause. We know of no independent healthcare BPO evidence for this yet, so it is a design to measure, not a proven method.

Emerging

Patient access overflow on voice and chat

Agents handle identity verification, routine questions and narrow booking when your team is busy. One major cloud provider lists voice appointment management as preview only, so AI agents should start with routing and verification.

Early

Agents working claims across client systems

Agents working claims and appeals across systems, with audit trails, are being piloted. Bessemer and Bain report 67% of provider RCM respondents using semi- or fully autonomous agents, but that is self-reported.

What stays human

Responsibilities that stay with your people

Delivery lead or account manager

Owns the client service levels, explains exceptions and decides when work returns to the client.

Quality auditor

Samples completed work, defines error categories and has authority to pause an automated step.

Certified coder or appeals specialist

Approves codes and appeal content before submission, so accountability stays with a qualified person.

Privacy or compliance officer

Keeps business associate agreements, the subcontractor list and the incident process current as part of AI governance.

Team leader for the exception queue

Works the cases AI could not resolve and reports patterns back.

Improvements

Operational changes you can expect to work on

01

Lower exception and rework rate

Measure the share of items needing a human fix and the reasons, then remove the common causes.

02

Shorter turnaround per work item

Track queue age and receipt-to-completion time by client and task type.

03

Client reports that build themselves

A weekly pack of volumes, accuracy and escalations can replace manual status calls.

04

Faster onboarding of a new client workflow

A written playbook for rules, systems and sampling is meant to reduce surprises on day one.

Commercial value

Winning and keeping outsourcing contracts

Buyers outsource to reduce cost and uncertainty, and they stay with vendors who show quality and controls. AI changes your cost to serve, and go-to-market work turns the delivery model into a story procurement can check.

  • Stand out in procurement with a documented human plus AI model, sampling plan and incident process.
  • Move selected services toward per-outcome pricing where your baseline shows stable volumes and error rates.
  • Expand inside accounts by adding adjacent tasks once a first workflow has proven its exception rate.
  • Retain clients with reports on turnaround, accuracy and escalations rather than activity.
Rules and limits

US and UK rules that apply, and what is not ready yet

General information for planning, not legal advice. Statuses and dates are as published at the time of writing.

United States

  • HIPAA business associate rules apply now. HHS guidance says an AI-enabled contractor handling PHI for a covered entity can need a business associate agreement and safeguards; the OCR and HHS materials we cite do not set out a separate general HIPAA AI rule.
  • Texas SB 1188 took effect 1 September 2025, and its EHR storage provisions apply from 1 January 2026, including older records. Covered EHRs, cloud-held ones included, must be physically kept in the US or a US territory. Our sources do not address remote access from abroad.
  • The HIPAA Security Rule proposal published 6 January 2025 is not final, and the 2026 regulatory agenda gives it a July 2027 target. Do not present it as an existing requirement.
  • The FCC ruled on 8 February 2024 that AI-generated voices are artificial voices under the TCPA, so outbound AI calls need prior consent unless an exception applies.

United Kingdom

  • ICO guidance expects procurement diligence, assessed controller and processor roles and documented subcontractor arrangements for outsourced AI handling personal data. Audit model access, data reuse, security and error correction first.
  • UK GDPR still needs an Article 6 lawful basis and an Article 9 condition for health data. The Data (Use and Access) Act 2025 amended UK GDPR; it did not replace it.
  • Organizations accessing NHS patient data or systems use the Data Security and Protection Toolkit, with a 2025 to 2026 deadline of 30 June 2026.
  • This page does not cover UK rules on sending personal data offshore, because we have not verified them here. Take that question to the client's data protection officer and qualified counsel.
Not mature or not suitable yet
  • Touchless claims processing as a promise. Most surveyed buyers have not yet put AI into their revenue cycle, and agentic claims work depends on audit trails and exception handling you must build.
  • Autonomous medical-necessity denials. This is high risk, and California SB 1120 requires a licensed professional to make medical-necessity determinations for covered health plans.
  • Quoting return on investment from self-reported surveys. Independent, BPO-specific performance evidence is thin.
Where to start

Where we would begin

  1. 01

    Baseline one client workflow

Sample today's manual work to record turnaround, error rate and exception reasons before any automation changes them.

  2. 02

Map data, vendors and storage locations

List every system, AI vendor and agreement that touches client data, and check where records are stored and who can see them.

  3. 03

Run a supervised pilot with a quality sampler

Automate the safest step with an exception queue and human approval, then share results with the client.

Questions

Frequently asked questions

Does using AI change our HIPAA obligations as a vendor?

It can add to them. If your service creates, receives, maintains or transmits PHI for a covered entity, business associate requirements can apply, including an appropriate agreement and safeguards. Assess each AI vendor you use the same way.

Should we move from per-hour to per-outcome pricing?

Only where your data supports it. We found no independent data on how healthcare BPO contracts are being repriced, so we model it from your own volumes, exception rates and rework. Outcome pricing without a stable baseline moves risk onto you.

Can we keep processing client data offshore?

That depends on the client contract and applicable rules. Texas requires covered EHRs to be physically kept in the US or a US territory from 1 January 2026. We map where data is stored and accessed before designing the workflow.

How do we prove AI-assisted work is accurate?

Sample it. Each week a qualified person checks a risk-weighted selection of completed items and you report error rates and exception reasons. Keep the source document and AI output linked so any item can be traced.

Will AI reduce the number of staff we need?

Possibly, but the evidence we have describes intentions, not outcomes. A common first change is a shift from keying to review and exception handling, and we plan those role changes with your managers.
